For you to enjoy all the benefits of concierge medicine without upfront fees and contracts, we do not bill insurance companies directly for your care. This involves much time and staffing, reducing the attention you receive during and after your office visit. As a service to those who are employed by the Memorial HealthCare System and the US Military, we will do the billing for you and your dependents.

Although we require payment from you at the time of service, you will be provided with a comprehensive bill that includes all the codes for treatment and the amount you paid. This should be submitted to your insurance company for reimbursement.  Most insurance companies will pay you back 60-80% of the cost of your visit. Call your insurance company to find out your deductable and out of network coverage.  We can check this coverage for you, if you provide us with the insurance information.

If you have insurance, any referrals, lab tests, and radiology exams will be billed directly to your insurance company. The only out-of-pocket expense for you will be the cost of our office visit. After reimbursement, the final cost of this visit will be about the same as the copay you would have with an in-network provider.

Office visits start at $50 for established patients.

The cost starts at $100 for new patients, yearly check ups and GYN exams. 

After hours and weekend visits start at $75.

Treatment given over the phone, that saves you an office visit, costs $25.

Physical exams for school, camp or sports cost $35.

DOT, pre-employment and other physical exams cost $50.

Forms that need completion after an office visit cost $10 to fill out. 

A visit for a vaccination only, costs $25, plus the cost of the vaccine.  

There can be additional costs for lab tests, EKGs, medications, supplies or vaccines given during your office visit.
