This link will allow you to view your medical record, obtain lab results, request prescription refills, make appointments, leave messages for your doctor and more.  

If you do not have a user ID and password, simply click the link above and complete the registration information. You will receive your login information by email within 1 business day. The message may be in your junk mail folder.

When you sign in for the first time, you will be prompted to change your password. Passwords need to be 8-10 characters long and include 1 number. For medical questions and messages for your doctor, please use the patient portal.

Before your first visit to the office, you may want to click the link above and fill out the PATIENT FORM. Once you have completed the form, hit the SAVE button, and then hit the SUBMIT button.  Now all your past medical history is available for the doctor to review. You may also want to complete the patient information section.  This will save you time checking in with the receptionist when you arrive in the office.

This link will allow physicians involved in your care to view your medical record, obtain results of labs and tests already performed, and make follow up appointments for you.  Consulting physicans can also communicate their suggestions for further treatment to your primary doctor.

Physicans who do not have a user ID and password should call the office or email us your request.
